Membership Renewal

Payment of dues and forms must be submitted by March 31. 

If you are renewing your membership, dues must be paid using this form. If you have technical difficulties, contact Kay to pay by check and submit hard copy forms.

Membership dues are $50 for the member-year April 1 through March 31.  There is no prorating of dues.

Membership Acknowledgement

By-laws Article II section 2B requires you read and sign this form if you agree to the following:
Any person, eligible for membership under Article II of these Bylaws, is qualified for membership only after such person agrees to sign an acknowledgement form as part of the application for membership indicating the member’s agreement of the following:

  1. Agrees with the purposes and goals of the Corporation;
  2. Agrees to abide by these Bylaws of the Corporation;
  3. Agrees to abide by written Policies and Procedures as enacted by the Board of Directors;
  4. Agrees not to have any interest or property right in the assets of the Corporation;
  5. Agrees to the following responsibilities
    1. To maintain humane and loving conditions for all rescued cats.
    2. To not impugn the integrity of the corporation by actions or words;
    3. To follow established procedures for resolving internal grievances;
    4. To not represent him/herself as a public spokesperson for the Corporation without prior Board of Director approval;
    5.  To not use the Corporation’s name, likeness, intellectual property rights or affiliation with or membership in the Corporation for personal gain.

Liability Acknowledgement

As part of my membership with Community Concern for Cats, Inc. (CC4C), I agree to the following:

  1. Assumption of Risk: I understand that activities involving cats carry inherent risks, including but not limited to scratches, bites, allergic reactions, illness, disease, or injury. These activities may include fostering, adoptions, providing cat medical care, participating in trapping, and rescuing cats. I knowingly assume all risks involved in handling, transporting, trapping, or rescuing cats.
  2. Release of Liability: I hereby release, discharge, and hold harmless Community Concern for Cats, Inc., its Board of Directors, officers, and members from any and all claims, demands, actions, or causes of action arising out of or related to any injury, illness, damage, or loss I may sustain as a result of activities involving cats through my involvement with CC4C.
  3. Safety and Conduct: I understand that following CC4C’s Policies and Procedures helps ensure safe practices and proper care in all aspects of cat care, rescue, and related efforts.
  4. Medical Insurance and Treatment: I understand that CC4C does not provide medical insurance or coverage for any injuries I may sustain during any volunteer or membership activities.

Your Contribution

The membership definition in the Bylaws states: Active members are expected to contribute some level of time and effort to achieve the goals and objectives of the Corporation. To renew your membership, indicate which activities you will volunteer for below.
